DecisionDx-UM test can only be ordered by a physician or other appropriate healthcare provider—usually an ophthalmologist with advanced training in ocular oncology or retinal surgery. Currently, the majority of ocular oncology specialists in the U.S. offer DecisionDx-UM to their patients. The test is performed on biopsy tissue from the primary tumor.

Testing process
Castle’s Clinical Services team coordinates the collection of the tissue sample from the pathology lab or clinic. Castle offers three tests for patients diagnosed with uveal melanoma that can all be run from a single fine needle aspirate biopsy (FNAB) of the primary tumor. If the treatment plan is enucleation, the tumor sample can be provided either from a biopsy taken at the time of enucleation, or from tissue slides from the wax block (FFPE) made following surgery.
All three of Castle’s uveal melanoma tests use tissue from a single biopsy or excision procedure, avoiding additional procedures.
Report results
The DecisionDx-UM test report provides actionable results to direct follow-up care. The test classifies patients according to their biological risk of metastasis over five years – low (Class 1A), intermediate (Class 1B) and high (Class 2). DecisionDx-PRAME and DecisionDx-UMSeq, when used in conjunction with the DecisionDx-UM test, may add further precision and help guide the most appropriate follow-up care regimens.

Castle Biosciences is committed to providing high-quality molecular testing to all patients. We do not want financial concerns to be a barrier to patients accessing critical healthcare information. We will work with all insurance providers, including Medicare, Medicaid, commercial insurers, and Veterans Affairs (VA), to secure payment coverage for our testing.
